Pupil leave of absence form, which we require a parent/carer, to complete if a child requires time away from school.

Parental agreement for school to administer medication. This form needs to be completed if your child requires medication at school to enable a member of staff to administer it. (For antibiotics, we would only administer if they were needed more than 3 times a day.)
